How to Participate
In order to certify patients for medical marijuana, you will need to be a:
Minnesota-licensed doctor of medicine.
Minnesota-licensed physician assistant acting within the scope of an authorized practice.
Minnesota-licensed advanced practice registered nurse who has the primary responsibility for the care and treatment of the qualifying medical condition of a person diagnosed with a qualifying condition.
If you are any one of those already, congratulations, you’re halfway there! In order to certify patients, you will need to enroll in the state’s Medical Cannabis Registry. To do this, you will need your:
Medical license number (not your NPI)
DEA number
Primary clinic address information
You will need to create an account with a username and password and complete the Health Care Practitioner Enrollment Form. You can do this here.
How to Certify Patients
The process is pretty straightforward, actually. You will need to conduct a physical exam of your patient and review their medical records. Make sure to also review their diagnostic tests and lab results in order to confirm their diagnosis.
At this point, you should create an ongoing treatment plan for your patient’s qualifying condition.
You will need to get your patient’s Email and Acknowledgement Form. You will then log into the registry, complete their certification online, and submit this information. The Office of Medical Cannabis will email your patient with instructions.
